Abstract

Non-alcoholic fatty liver disease (NAFLD) is a prevalent liver disorder that can progress from simple steatosis to steatohepatitis and fibrosis. Curcuma longa L. and its bioactive compound, curcumin, have been investigated for hepatoprotective effects, but evidence varies across bioinformatics, in vitro, animal, and human studies. This systematic review aimed to synthesize evidence on the hepatoprotective effects and molecular mechanisms of Curcuma longa and curcumin in NAFLD, distinguishing preclinical from clinical evidence. Literature searches were conducted in PubMed, Europe PMC, and Google Scholar using terms related to Curcuma longa, curcumin, hepatoprotection, inflammation, and NAFLD, following PRISMA 2020 principles. Evidence suggests curcumin may reduce hepatic lipid accumulation and modulate oxidative stress, inflammation, insulin sensitivity, and gut–liver pathways. However, clinical evidence remains limited, with differences in formulations, doses, durations, and outcomes. Curcumin is therefore a promising investigational adjunct, but standardized clinical trials are needed to establish efficacy, optimal dosing, formulation, and long-term safety.
